How Does It Work?

Isotretinoin is the only acne treatment that targets all four major factors in acne pathogenesis:

1. Shrinks sebaceous glands dramatically reduces sebum (oil) production, often by 80–90%, which starves the skin environment that acne-causing bacteria need to thrive.
2. Normalizes follicular keratinization reduces abnormal shedding/clumping of skin cells inside hair follicles, which prevents the pore blockages that start comedones (blackheads/whiteheads).
3. Reduces Cutibacterium acnes colonization indirectly, since less sebum means less food source for the bacteria.
4. Anti-inflammatory effects reduce inflammatory mediators in the skin, calming existing lesions.

The exact molecular mechanism isn’t fully mapped, but it’s believed to work through retinoic acid receptors (RARs) that regulate gene transcription in skin cells.Isotretinoin like Accufine 20 mg, Accufine 30 mg, Saferet 30 mg, Tretiva 10 mg, Isotroin 20 mg, and Sotret 30 mg is to be used in dealing with acne and improving skin texture.

Efficacy

• Almost 85–90% of patients see substantial, often permanent, clearance after a single course (typically 15-20 weeks).
• Cumulative dosing (usually targeting 120-150 mg/kg total over the course) correlates with lower relapse rates.
• Relapse occurs in roughly 20-30% of patients, more likely with lower cumulative doses, younger age at treatment, or more severe baseline disease retreatment is possible.

Safety

It may show some side effects like Mucocutaneous dryness i.e., chapped lips (cheilitis) is nearly universal, dry skin, dry eyes, dry nasal passages (occasional nosebleeds), Photosensitivity sunburns more easily, Temporary acne flare in the first few weeks, Joint/muscle aches, Elevated liver enzymes, triglycerides, and cholesterol (requires monitoring), Thinning hair (usually temporary).

Precautions

• Teratogenicity (birth defects), this is the biggest concern. Isotretinoin causes severe fetal malformations (craniofacial, cardiac, CNS) if taken during pregnancy even a single dose.

• Two negative pregnancy tests before starting, monthly tests during treatment.
• Two forms of contraception required (or abstinence) for patients who can become pregnant, starting 1 month before treatment through 1 month after
