Paroxetine Hydrochloride is SSRIs (Selective Serotonin Reuptake Inhibitors) it is used in dealing with premature ejaculation and also used as an antidepressant, in dealing with mood and anxiety conditions like Major Depressive Disorder (MDD), Obsessive compulsive Disorder (OCD), Post Traumatic stress Disorder (PTSD) and Premature Dysphoric Disorder (PDD).

Paroxetine may work by:
- Paroxetine works by increasing the amount of serotonin available in the brain. Serotonin is a neurotransmitter that helps regulate mood, anxiety, sleep, and obsessive thoughts.
- Normally after serotonin is released between nerve cells, it gets “reabsorbed” back into the neuron by a serotonin transporter.
- Paroxetine selectively blocks this transporter, so more serotonin stays in the synapse for longer.
- Over time this leads to better communication between brain cells in areas that control mood and fear. Compared to other SSRIs, paroxetine is also mildly sedating and has some anti-cholinergic activity, which makes it especially useful for anxiety, panic, and OCD.

Safety & Efficacy
- Clinical studies show paroxetine is effective in about 60-70% of patients with depression and is considered one of the most effective SSRIs for panic disorder and social anxiety. For OCD it often requires higher doses and 8-12 weeks to see benefit.
- Common side effects include nausea, headache, drowsiness or insomnia, dry mouth, sweating, and sexual side effects like decreased libido or delayed orgasm. It is also more associated with weight gain than other SSRIs. Serious but rare risks include Serotonin Syndrome if combined with other serotonergic drugs, increased suicidal thoughts in people under 25 during the first few months this carries an FDA Black Box Warning, bleeding risk when taken with NSAIDs or blood thinners, and low sodium hyponatremia in the elderly.
Precautions
- Paroxetine should not be used with MAO inhibitors like phenelzine or within 14 days of stopping them due to risk of fatal serotonin syndrome.
- It also should not be combined with thioridazine or pimozide. Caution is needed with other antidepressants, tramadol, triptans, and St. John’s Wort.
- It is a strong CYP2D6 inhibitor, so it can raise levels of drugs like tamoxifen, metoprolol, and atomoxetine.
- Paroxetine is not recommended in pregnancy, especially in the first trimester and late 3rd trimester, due to risks of birth defects and new born complications.
- It passes into breast milk. People with liver or kidney disease, history of seizures, bipolar disorder, or angle-closure glaucoma need dose adjustments and close monitoring. Older patients have increased risk for falls, sedation and low sodium.
